Abstract
Neuroleptic malignant syndrome (NMS) refers to the combination of hyperthermia, extrapyramidal symptoms, altered consciousness and autonomic symptoms that can occur as a serious complication of the use of antipsychotic drugs. Donepezil-related NMS was rarely reported. We report a patient with dementia with Lewy bodies (DLB) who developed NMS following donepezil monotherapy. A 74-year-old female with DLB visited to our hospital with confusion, fever and severe muscle stiffness for 2 days. Four days before admission, she was started on donepezil to treat dementia. This report suggests that caution should be used when starting donepezil in patients DLB.
